Tattoo Waiver

I acknowledge by signing this agreement that I have been given the full opportunity to ask all questions which I might have about the procedure.

I specifically acknowledge I have been advised of the facts and matters set forth below and I agree as follows:

• If I have any condition that might affect the healing of this tattoo, I will inform my artist

• I acknowledge it is not reasonably possible for the shop or employee to determine whether I might have an allergic reaction to the pigment used in my procedure, and agree to accept the risk of such a reaction is possible.

• To my knowledge, I do not have a physical, mental, or medical impairment or disability which might affect my wellbeing as a direct or indirect result of my decision to get tattooed.

• The nature and method of the proposed procedure have been explained by the technician as any risks or complications during and following its performance

• I acknowledge that the requested procedure(s) was applied following universal precautions as outlined by the CDC and my risk of infection begins the moment I leave the facility

• I understand that a certain amount of pain or discomfort is associated with this procedure

• I understand if I do not follow the aftercare instructions that were given and, as a result, get an infection, I will not hold NerdyNeedleTattoosLLC or any of its employees at fault. I acknowledge that I am over the age of eighteen and that I have truthfully represented to my artist that the obtaining of the tattoo or piercing is by my choice alone. I consent to the application of the tattoo and to any actions or conduct of the representatives and employees of the tattoo shop reasonably necessary to perform the tattoo/piercing procedure.
